Issaquah Plastic Surgery Medical Records Requests

Dear Valued Patient,

I will be retiring from professional practice as of April 30, 2024. After 25 years of patient care, I am stepping back to care for family and for adventures that are taking me outside of Issaquah. I am honored and privileged that you allowed me to care for you during this time.

Sometimes we need a copy of our archived medical records. If your surgery was at Swedish, operative reports can be obtained via MyChart or by completing an Authorization for Disclosure of Health Information to obtain your records. This form and the information about how to obtain your medical records can be found online at www.swedish.org/medicalrecords. If your surgery was at Overlake, operative reports can be obtained via MyChart or completing an Authorization for Disclosure of Health Information at https://www.overlakehospital.org/visit/medical-records

Your paper clinic chart will be kept in storage for ten years. A copy of relevant documents can be obtained by emailing records@issaquahplasticsurgery.com after 4/30/24. A search and copying fee as per WAC 246-08-400 may apply. Search fee: $26.00 Pages 1-30: $1.17 per page Pages 31+: $0.88 per page. After confirmation of identity and receipt of payment, records will be sent via mail.
